DISEASE MANAGEMENT AND WELLNESS PROGRAM
NetCare covers a variety of programs focused on Disease Management and Wellness Improvement:
- NetCare covers 80% of Contracted Rates with Participating Wellness Providers with members being responsible for 20% co-insurance. This applies to the clinic visits, laboratory procedures and medications.
- Deductible does not apply to Disease Management/Wellness programs.
- After completion of the program, member can request for reimbursement for the paid 20% co-insurance. The reimbursement claim should be submitted 90 days from the date of service.

7 Day Detox Programs
- Intermediate level program to help reset your lifestyle. Meals are provided with additional raw food and phytonutrients in the meals. This program is designed with whole food approach, lifestyle intervention, addressing sleep hygiene, stress management, hydration, mindful eating and physical activity.
- SDA provide all the meals and whole food powder mix for 7 days
- The cost of this program is $55 which is the 20% co-insurance.
7 Day Advance Detox Programs
- This program is also designed with whole food approach but utilizing fasting mimicking diet (all the benefits of fasting without the hunger) with emphasis on lifestyle intervention. Participants can still enjoy meals (this is not a liquid diet program) with guidelines provided while receiving high nutrient density food powder mix packed with powerful phytonutrients for 1 week.

- This program applies medical, behavioral, motivational, and environmental Principles to address lifestyle related chronic disease. This is an educational program focusing on lifestyle medicine, discovering the root cause of the chronic disease, utilizing evidence based lifestyle change as the first line therapy for disease treatment and reversal.
- For those members with DM, Pre-Diabetes, High Blood Pressure, High Cholesterol, Excess Weight, Gout, Kidney Failure
- The cost for this program is $94 which is the 20% co- insurance
DiaBeatit Program
- For those members with Diabetes Management or Pre-Diabetes.
- This is a certified program which meets all the quality standards by DEA approved by AADE.
- The cost for this program is $273 which is the 20% co-insurance
Shape Up
- This program is designed with whole food meals, lifestyle interventions addressing sleep hygiene, stress management, hydration, mindful eating and physical activity. Training materials will be provided to help participants improve their daily routine with emphasis on intermediate fasting.

This program, under the direct care, supervision and guidance of a Certified Nutrition Specialist (CNS) and Chiropractor (D.C.) - First week
Initial Consultation with Physician. Medical History/Physical, Labs, EKG, Discuss pharmaceutical and non pharmaceutical options. - Second Week
Initial visit with Dietician or Wellness Coach for Nutritious education and lifestyle management - Third Week
Follow-up with physician to discuss labs and implement medical plan. - Fourth and Subsequent Weeks
Additional 6-8 follow-ups with dietician, wellness coach and physician to motivate and encourage patient compliance to the program and make lasting change.
